Who provides your care

Peak Hair Restoration is a management company that provides nonclinical support to JC Jimenez Medical Health Wellness Services PLLC, including scheduling, billing support, administration, information technology, marketing, and patient coordination. Peak Hair Restoration does not provide medical advice, prescribe medications, or perform medical procedures.

JC Jimenez Medical Health Wellness Services PLLC provides all medical services. Its licensed clinicians are responsible for patient evaluations, treatment decisions, and surgical and nonsurgical hair restoration procedures.

Appointment policy

To help secure your desired date and time, we suggest booking a few weeks in advance. Please confirm current availability with Peak's scheduling team. A credit card is required to reserve an appointment; do not enter card details into the general website inquiry form.

Consultations policy

Peak coordinates free consultations. Evaluations of hair loss, donor capacity and treatment suitability are performed by licensed clinicians of JC Jimenez Medical Health Wellness Services PLLC, not Peak's scheduling team. The clinician can discuss your goals, realistic expectations, available options now and over time, and a proposed plan suited to your situation. There is a limit of two (2) free consultations per person in any 365-day period.

Cancellation and late-arrival policy

For individual appointments, please give at least twenty-four (24) hours' notice to cancel or reschedule. A $100 charge applies if you cancel or reschedule less than twenty-four (24) hours before your appointment or do not attend.

For group appointments, please give at least seventy-two (72) hours' notice. The card on file will be charged $200 if a group appointment is canceled less than seventy-two (72) hours in advance.

If you arrive more than ten (10) minutes late, you may be asked to reschedule to avoid disrupting other patients' appointments.

Pricing and prepayments

Prices for products and services may change at any time. Once prepaid, a purchased service must be redeemed within one (1) year of purchase.

If a client's credit or debit card or CareCredit payment, where applicable, has a chargeback, fraud issue or other payment problem, those payment methods may no longer be accepted from that client, and cash payment may be required.

Your individual treatment, total price and what is included should be confirmed in writing before you pay. Clinical recommendations and decisions come from the medical practice's licensed clinicians.

Returns and refunds

If you purchase a skincare product through Peak's services, you may return it in its original packaging, unopened, within thirty (30) days of purchase. We do not accept returns of opened or used products. Gift cards, where offered, cannot be returned or exchanged.

Prepayments are refundable within ten (10) days of purchase. Services already received cannot be refunded. Payments for training courses, if offered, are non-refundable.

Children and pets

Children

For children's safety and the comfort of patients, please arrange childcare while you receive services. An adult must accompany anyone under eighteen (18) receiving services.

Pets

For the health and safety of patients, pets are not permitted. Working service animals are welcome.

Indemnification

To the extent permitted by applicable law, you agree to defend, indemnify and hold harmless Peak Hair Restoration, its affiliates and subsidiaries, and their directors, officers, shareholders, managers, employees, agents, partners, representatives, licensors, successors and assigns (collectively, “Related Parties”) from a claim, demand, loss, damages, expense or cost (including reasonable attorneys' fees) arising out of:

  1. your use of, inability to access, or reliance on the website or services, or goods or services obtained through them;
  2. your use of Peak's nonclinical coordination services in connection with a treatment;
  3. your travel in connection with a treatment;
  4. your interaction with another user;
  5. a claim that you violated a provision of an agreement with Peak;
  6. a violation of such an agreement by you or another person using your account, if applicable, whether or not authorized by you;
  7. harm to a person or property resulting from your acts or omissions, whether intentional, negligent or otherwise;
  8. Peak's use, disclosure or preservation of content you submit, subject to applicable privacy law; or
  9. your violation of another person's rights, including intellectual property rights.

This website section concerns Peak's nonclinical services; medical care is provided by JC Jimenez Medical Health Wellness Services PLLC under the applicable clinical documents. Nothing in this section waives rights that cannot legally be waived or limits the medical practice's or Peak's responsibilities under applicable law.
